This is the dashboard for your results from the P3C-OCT (Person Centred Coordinated Care (P3C) - Organisational Change Tool (OCT)).

Each section will have graphs similar to the one below. The graph below displays the overall score of your organisation on the P3C-OCT in red, compared to other organisation scores are in blue. Higher scores indicate stronger P3C (Patient Centred Coordinated Care).
